MEDICAL CODING

             Medical coding is the process of assigning medical codes to documentation in a patient's medical record for reimbursement and data collection purposes.

Roles and Responsibilities of a Medical Coder :

1. Review medical records to ensure accuracy and completeness of patient data.

 2. Assign appropriate codes to diagnoses and procedures.  

3. Identify discrepancies in patient records and make corrections as needed. 

4. Generate patient claims and billings in accordance with insurance requirements. 

5. Verify accuracy of codes, and ensure that all coding is compliant with government regulations. 

6. Maintain a current knowledge of coding practices, coding systems, and coding software. 

7. Communicate with physicians and other healthcare team members to resolve coding questions and ensure accuracy of coding. 

8. Assist with internal and external audits, as needed.

 9. Prepare reports on coding and billing activities. 

10. Create and maintain coding databases and records.
